Sign-up for a Take Charge of your Health - Live Well Be Well Workshop (workshop formally known as the Chronic Disease Self-Management Program or CDSMP).   This workshop is designed for adults age 55 and older who want to take charge of their on-going health problems and get relief from their pain, fatigue, and other symptoms.
